MyDHR is an online patient portal giving you access to your health information. If you are a current patient of ACT Health, you can request a MyDHR account. Some of the features and functions available to you through your MyDHR account include: Pre-arrival tasks - Complete the pre-arrival tasks from home to save time when you arrive at the clinic. Health and Visit Summaries - View, download, or send a copy of specific visit summaries from your health record. Questionnaires – Fill in questionnaires prior to your visit. Request Health Information - Request a copy of your medical record and download it when it is available. Test results – view pathology and imaging test results. Current Health Issues - Review your current health issues and update them as needed. Advance Care Planning – Review and upload documents, such as advance directives and a living will. Organ and tissue donor registration - Registering to be an organ and tissue donor is easy. Appointments – View appointments and receive reminders. Proxy access – Access your family’s health information through proxy access. Apple Health - Connect your account to Apple Health to pull your health-related data form your personal device straight into MyDHR.
